Purpose of Use of Personal Information

• [1] Conducting medical examinations and tests to assess health conditions
• [2] Sending health checkup results
• [3] Billing for health checkup services
• [4] Providing notifications for subsequent health checkups

(Please note: Item [4] will not be carried out if the customer does not consent.)

Personal information will only be used for the purposes stated above. If it is necessary to use personal information for other purposes, we will explain the reason to the individual and obtain prior consent.

Provision of Personal Information to Third Parties

Except where required by laws and regulations, the Foundation will not provide personal information to any third party without obtaining prior consent from the individual.

Reporting of Health Checkup Results

Health checkup results (including optional tests) will be reported to the individual. In addition, based on requests from contracted organizations (such as companies and health insurance associations), results may be provided to those organizations.
(However, this does not include HIV test results.)

Outsourcing of Operations

In order to carry out health checkup services, the Foundation outsources part of its operations to external contractors.
We supervise such contractors through contracts and other measures to ensure proper management of personal information.
The main outsourced operations include:

• • Information system management
• • Comprehensive security services

Rights of the Individual

Individuals have the right to request disclosure, correction, deletion, or suspension of use of their personal information held by the Foundation.

Voluntary Provision of Personal Information

Providing personal information to the Foundation is voluntary. However, if required information for health checkups (such as name, date of birth, address, and affiliated health insurance association) is not provided, we may not be able to offer health checkup services.

Accredited Personal Information Protection Organization

An Accredited Personal Information Protection Organization is an entity authorized by the competent minister under Article 37 of the Act on the Protection of Personal Information. It ensures the proper handling of personal information by processing complaints and providing information to member organizations.
The Foundation is a member of the following accredited organization:
